Physician Signature
Why do I need a physician signiture for my child’s health form?
Answer
A physician’s signature is required (by NY State) on the health form for our trained camp personnel to administer ANY medications to campers. This includes over the counter medications as well as their prescription medications.
If you do not wish for your child to receive any medication, then the signature is not necessary.
You can request your child’s doctor’s office to sign the health form, attach the immunizations and fax them into camp.
